Support Coordination

We are 'Human-Centered' and 'Solutions-Focused', working collaboratively with 'Informed-Consent', in the co-design and implementation of disability services and supports required for individual and sector connection. In doing so, we take a 'Strengths-Based' and 'Wrap-Around' approach.

We are professionally educated, trained, and independent NDIS specialists available to participants of the NDIS who are approved for a plan and funding that is either 'Plan Managed' or 'Self Managed', not 'NDIA Managed'.

Testimonials

“Within 24hrs of contact Rhys addressed all my inquiries related to my son’s NDIS plan and funding. He picked up a number of errors related to funding buckets, plan management type and assisted me to address officially and correct with the NDIS planner. Rhys’s NDIS knowledge is extensive and he knew what to do straight away. What he assisted me to address on behalf of my son was done within 24hrs whereas the three months prior no one could answer any of my questions, let alone properly assist me and my son regarding navigating and understanding the NDIS, funding, plan implementation and usage.”

Mother of  ASD high school student, Central Highlands

“We were very glad to have Rhys work with us, where he spent time getting to know our child and their needs, then matched them with an excellent support worker.

Rhys is knowledgeable and informative, and his friendly, down-to-earth approach makes him easy to work with and easy to understand.

Thank you very much Rhys!”

Your Questions Answered:

Via ‘Supported Decision-Making’, we map and plan with the participant and those important to them. We identify what is best required to have in place regarding paying from the NDIS plan for services and supports, as well as what is not required to pay for that we can assist the participant to connect with, against the goals of the NDIS plan.

We collate all this for review and approval and then communicate this out to all those involved, with participants documented informed-consent, in best ensuring NDIS plan goals are worked towards and achieved. We are happy to address through this via our ‘thirty-minute’ free consultation session located on the ‘Contact Us’ page.

With the NDIS, every participant has a contact person (details of this person are contained within the plan itself), who overall is responsible for the participant and the plan taking effect. We ensure that for all paid and approved works we do with the participant that are of importance, this NDIS contact will have all appropriate documentation sent through to them (with participants prior informed-consent) – with the request, it is reviewed and forms part of the participants overall customer file they have with the NDIS. As for the participants paid chosen supports and services, we do this via what is known as a ‘service agreement’, that is put in place prior to any commencement of these supports and services. We are happy to take you through this via our ‘thirty-minute’ free consultation session located on the ‘Contact Us’ page.

With the participants prior informed consent, these important people will be contacted and invited to be involved within the mapping and planning session we will all do together. We can also arrange for their ongoing involvement and communication that ensures the participant will have them at their side all the way through the journey. We are happy to take you through this via our ‘thirty-minute’ free consultation session located on the ‘Contact Us’ page.

We have a combined total of over fifty years demonstrated worked and lived experiences (disability and mental health). We are happy to address this through via our ‘thirty-minute’ free consultation session located on the ‘Contact Us’ page.

  • We assist you to identify what formal services and supports you require your NDIS budget to pay for. We also determine what informal services and supports are available and accessible for you as well that don’t require your budget to pay for. We plan the implementation and ongoing usage of overall services and supports in your daily life, routines and against schedules as required by yourself against your NDIS plan timeframe and budget.
  • We ensure that the services and supports paid for via the NDIS plan are all above board and have the relevant checks, clearances, and expertise in providing services and supports for the participant.
  • We also monitor-review and evaluate the effectiveness of supports and services to insure the participant is getting what they pay for, and that progress reports and required NDIS reporting is all in place and adhered to.
  • We are here to assist with troubleshooting and providing ‘supported decision making’ solutions to any problems the participant may experience in receiving services and supports paid for, as well as assisting with all NDIS plan and communication related matters.
